Dynamic Absence Record with Google Sheets

This form collects absence records that must be provided monthly to the administrative office. It integrates with a Google Sheet to dynamically initialize a pick list with a list of school sites. When the user picks a school site, the form retrieves additional information relevant to that school site from the Google Sheet.

The form also uses dynamic behavior, automatic calculations and conditional controls.
